Notice for NY F08 Prospective Students

If you have received an offer letter and will be accepting your seat for the Fall 2008 semester, you must fax back your signed offer to the fax number listed on your letter. You will also need to return the Reimbursement form that was included with your offer letter. This form must be signed by you and your supervisor/manager at Verizon. If you have misplaced our Reimbursement form, please click the below link to print another copy. *Please note - this form should only be submitted by students that have been offered a seat into the Program. If you are unsure of your status, you may go to ASK GUS (link located at the top of this page) to review your current program status.

If you have verified your status for Fall 2008 in ASK GUS and it indicates that you have not completed your Authorization form or that the form we have on file is not for the school you have applied to for Fall 2008, you will need to sign and return a current copy of the Authorization form.
